• In-Person position with future opportunities for remote
• Shift: Monday through Friday from 8:30 AM - 5:00 PM
• Communicates with patients effectively, providing information and addressing concerns through various channels
• Coordinates patient care by managing referrals, follow-ups, and collaborating with healthcare providers

• Promotes a positive patient experience by demonstrating caring behaviors, maintaining clear communication, and collaborating with the care team to provide safe, timely, and effective whole person care
• Ensure the appropriate use and stewardship of medical supplies and equipment to support patient safety and operational efficiency
• Serves as a patient advocate, ensuring patient rights and needs are respected and addressed
• Administers immunizations according to established protocols and guidelines
• Completes medical documentation timely, accurately, and thoroughly
• Conducts patient intake by documenting patient information, including medical history and current medications
• Orders and maintains inventory of medical supplies, ensuring availability for patient care
• Other duties as assigned
