Licensed Practical Nurse - Hospice

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide bedside care and support to terminally ill patients in a hospice setting. This role focuses on delivering comfort, dignity, and quality of life during end-of-life care.

Key Responsibilities

• Provide direct bedside nursing care to hospice patients.
• Monitor and record vital signs, symptoms, and overall patient condition.
• Perform wound care, catheter care, and other basic nursing procedures.
• Assist patients with activities of daily living (bathing, dressing, feeding, toileting).
• Assess pain levels and implement comfort-focused interventions.
• Observe and report changes in physical, emotional, and mental status.
• Communicate patient updates to the RN, physician, and hospice care team.
• Educate patients and families on medications, care plans, and disease progression.
• Provide emotional support and reassurance to patients and families.
• Participate in interdisciplinary care team discussions.
• Maintain accurate and timely documentation of care and patient responses.
• Follow hospice protocols, safety standards, and infection control guidelines.

• Current Licensed Practical Nurse (LPN) license.
• Experience in direct patient care, preferably in hospice or palliative care.
• Strong communication and interpersonal skills.
• Ability to provide compassionate, patient-centered care.
• Must have at least 2 years of recent experience in the specialty within the past 2 years.

• CPR certification required.
• Additional hospice or palliative care certifications preferred.
