Major Responsibilities:
- Conduct a physical therapy evaluation of each patient and establish a plan of care and treatment goals. Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integument evaluations utilizing appropriate methods. Evaluate outcomes within an appropriate time frame.
- Provide ongoing assessment of patient response to treatments and teach and apply interventions as appropriate. Update and revise plan of care as indicated and provide the physician with a status report on a regular basis.
- Communicate problems to physicians and management and clearly state plans, actions, and goals so changes are reflected as they occur. Maintain and update clinical and patient records according to state and federal guidelines.
- Manage assigned caseload including scheduling and informing site leadership of coverage needs and ability to provide coverage. Manage patient medical conditions including medications and act as a patient advocate to insure optimal outcomes. Review caseload for Low Utilization Payment Adjustments (LUPA), optimal number of visits based on progress, Prospective Payment Plan (PPS), and Outcome Based Quality Initiatives (OBQI) outcomes.
- Manage a caseload of Physical Therapist Assistants (PTA) including evaluating their patients, ensuring compliance with Care Plan, and adhering to all Wisconsin regulations regarding PTA supervision.
- Perform Home Health Aide joint visits as appropriate to maximize patient functional abilities in the home. Assist with evaluating patients, turning patients over to PTA, and informing PTA of Plans of Care per Wisconsin regulations. Teach appropriate treatment procedures and supportive activities to patients and caregivers and develop plans for obtaining adaptive devices as needed.
- Discuss caseload issues and changes in case census with management. Communicate problems to physicians and management, and clearly state plans, actions, and goals so changes are reflected as they occur.
- Participate in chart audits and continuous quality improvement activities. Facilitate discharge planning process and utilize interagency/system and community resources to assure continuity of care after discharge.
- Act as a resource to employees, including training/orientation, mentoring, and providing guidance on complex issues. Serve as an instructor for students, meeting all expectations of the Clinical Student Program.
- Perform various patient positioning and transporting duties, which require lifting and pushing/pulling while utilizing proper technique.
- Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Demonstrate knowledge of growth and development principles over the lifespan and possess the ability to assess data reflective of the patient’s status and identify each patient’s requirements relative to age‑specific needs.
Required:
- Physical Therapy License in the state of Illinois or Wisconsin, required in the state of work.
- Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless waived).
- A valid driver’s license issued by the Division of Motor Vehicles.
Required:
- Doctorate degree in Physical Therapy from an accredited school or equivalent educational requirements.
- 1 year of clinical experience or demonstration of a strong spectrum of clinical affiliation experience.
Required:
- Knowledge of indications and contraindications of modalities and exercise.
- Skill in use of modalities, exercise equipment, and other available devices.
- Proficiency in clinical skills with the ability to work independently and make sound judgments.
- Good human relations, organization, problem‑solving, and decision‑making skills.
- Basic proficiency in Microsoft Office (Excel, PowerPoint, Word) or similar products.
Working Conditions:
- Obtain transportation for daily schedule of home visits and other business in a timely manner.
