Salary: $80,000 – $86,000 per year + mileage
Schedule:
Mon‑Fri 8 am‑5 pm + on‑call rotation

Qualifications- Associate’s degree (preferred)
- Registered Nurse license in the state(s) of practice (required)
- Current and valid CPR certification (required in Georgia)
- Minimum one year of experience (required in California)
- Previous experience in hospice, home health, general medical‑surgical or patient population‑specific nursing (e.g., geriatrics, pediatrics, etc.)
- Conduct comprehensive nursing assessments and develop individualized care plans with the interdisciplinary team, addressing physical, emotional, social, and spiritual needs.
- Maintain timely and accurate documentation of care, interventions, and patient status updates, ensuring compliance with internal protocols and regulatory requirements.
- Coordinate care activities including visit scheduling, medication management, physician collaboration, and interdisciplinary communication to ensure continuity and quality of care.
- Provide education and support to patients and caregivers, and respond to urgent needs during on‑call or admissions assignments.
- Supervise LPNs, LVNs, and hospice aides as needed, and participate in interdisciplinary team meetings, patient conferences, and ongoing clinical education.
- Support pre‑hospice services by evaluating patient status via phone, telehealth, or in‑person visits and identifying needs for additional care or services.
