Position: Home Care Registered Nurse (RN)

Description

We are seeking a compassionate and skilled Home Care Registered Nurse (RN) to deliver high-quality, personalized nursing care to patients in the comfort of their own homes. The ideal candidate is an independent, detail-oriented professional who thrives in a one-on-one care setting and is committed to improving patient outcomes and quality of life.

Key Responsibilities
  • Conduct comprehensive in-home patient assessments and develop individualized care plans
  • Administer medications, injections, IV therapy, and wound care as prescribed
  • Monitor vital signs, disease progression, and response to treatment
  • Educate patients and family caregivers on diagnoses, medications, and self-care techniques
  • Coordinate care with physicians, therapists, social workers, and other members of the interdisciplinary team
  • Document patient visits accurately and timely in the electronic medical record (EMR)
  • Ensure compliance with HIPAA, OSHA, and all state and federal home health regulations
  • Respond to changes in patient condition and communicate concerns to the care team promptly

Schedule: Flexible hours including days, evenings, and weekends based on client needs

Requirements Qualifications

License:

Current, Wisconsin Registered Nurse (RN) license.

Experience:

Minimum of two (2) years of nursing experience.

At least one (1) year of experience in home health or in serving individuals with developmental disabilities, physical disabilities, or the elderly.

Skills:

Proficiency in computer applications, including Microsoft Office, Adobe and electronic health record (EHR) systems.

Strong written and verbal communication skills.

Ability to conduct thorough in-person assessments while adapting to diverse populations and environments.

Strong organizational skills with attention to accuracy and detail.
