LTSS Service Coordinator (Case Manager)

Hiring near Southwestern Wisconsin in the following counties:
Pepin, Buffalo, Trempealeau, Jackson, Clark, La Crosse, Monroe, Juneau, Vernon, Sauk, Richland, Crawford, Grant, Iowa, Lafayette, Green.

Field:
This role enables associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands‑on engagement. Alternate locations may be considered if candidates reside within a commuting distance from an office.

Elevance Health offers the Family Care Program, a Wisconsin long‑term care program for older adults and adults with disabilities. The goal is that each member experiences the life they choose with supports to maximize independent living, employment, and community contribution.

The LTSS Service Coordinator is responsible for conducting service coordination functions for a defined caseload of individuals in specialized programs. In collaboration with the person supported, the coordinator facilitates the Person Centered Planning process that documents the member’s preferences, needs, and self‑identified goals—including assessments, development of a comprehensive Person Centered Support Plan (PCSP) and backup plan, interface with Medical Directors and participation in interdisciplinary care rounds to support integrated care planning.

The coordinator engages the member’s circle of support and overall manages the individual’s physical health (PH), behavioral health (BH), and LTSS needs per applicable state law, contract, and federal requirements.

How You Will Make An Impact
  • Perform face‑to‑face program assessments using various tools and pre‑defined questions, apply motivational interviewing techniques, and coordinate an individual’s waiver (e.g., Family Care Partnership) and BH or PH needs.
  • Use tools and pre‑defined identification processes to identify members with potential clinical health care needs, coordinate those members’ cases with the clinical healthcare management and interdisciplinary team, and provide care coordination support.
  • Manage non‑clinical needs of members with chronic illnesses, co‑morbidities, and/or disabilities to ensure cost‑effective and efficient utilization of long‑term services and supports.
  • Document the member’s short and long‑term service and support goals in collaboration with the member’s care team, which may include caregivers, family, natural supports, service providers, and physicians.
  • Serve as mentor, subject matter expert, or preceptor for new staff, assist in formal training of associates, and participate in process improvement initiatives.
  • Submit utilization/authorization requests to utilization management with documentation supporting and aligning with the individual’s care plan.
  • Report critical incidents to appropriate internal and external parties such as state and county agencies (Adult Protective Services, Law Enforcement).
  • Assist and participate in appeals or fair hearings, member grievances, appeals, and state audits.
Minimum Requirements
  • BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience that provides an equivalent background.
Preferred Skills, Capabilities and Experiences
  • BA/BS degree in a health care related field.
  • Assisted living or social work experience.
  • Waiver experience.
  • Comfortable using technology.
  • Travel to worksite and other locations as necessary.
