Position SummaryWe have an exciting opportunity to join our team as an Ambulatory Care Nurse (37.5 hours).
Role DescriptionThe Ambulatory Care Nurse supports the coordination, integration, communication, implementation, and evaluation of nursing practice and clinical policies and procedures. The nurse participates in site and organizational quality and performance improvement activities, collects patient health data using appropriate assessment techniques, and collaborates with licensed providers to develop patient-centered plans of care. The role includes administration of treatments and medications, patient safety monitoring, health teaching and counseling, disease prevention guidance, rehabilitation care, public health care, and supportive care for symptom relief.
Job Responsibilities- Performs physical examinations and diagnostic tests
- Collects patient health history
- Records information in the electronic health record
- Provides information, counsel, and education to patients and families regarding test results and treatment plans as directed
- Administers medications (including vaccination, IV therapy, oxygen administration), provides comprehensive wound care, and performs phone and in-person patient triage for clinical concerns
- Assesses and monitors patient responses to various treatments, procedures, and anesthesia
- Assesses and interprets patient information, making treatment decisions as appropriate
- Consults with clinical supervisors and licensed providers to determine best treatment plans for patients
- Directs, supervises, and delegates to licensed practical nurses, medical assistants, and technicians as needed to support patient flow and efficiency
- Performs other duties as needed
- Supports coordination and performance evaluation of nursing practice and clinical policies and procedures
- Ensures clinical compliance with patient safety initiatives and reporting
- Supports key quality initiatives to impact clinical outcome measures
- Demonstrates knowledge of NYULH service standards and incorporates them into responsibilities
- Participates in activities related to NYULH clinical standards and outside agency regulations
- Supports the missions, philosophy, standards, goals, and objectives of NYULH and Nursing
- Collaborates with interdisciplinary team and promotes collegial relationships
- Serves as a resource for nursing, other clinical staff, and providers
- Participates in clinical inquiry and evidence-based practice
- Maintains patient confidentiality and adheres to ethical obligations
- Provides emergency care per organizational policy
- Functions as a respectful member of the health care team. Delegates and oversees nursing interventions as appropriate
- Engages in self-assessment and professional development
- Maintains current nursing knowledge and competence
- Applies new knowledge to clinical practice and utilizes learning resources
- Mentors peers and upholds professional conduct
- Identifies and reports professional misconduct appropriately
- Executes medical regimens prescribed by licensed providers
- Assesses patients, identifies needs, and plans comprehensive care
- Identifies co-morbidities or complications and reports to the provider
- Coordinates care with other health care providers
- Provides direct care including administration of treatments and medications, health teaching, and counseling
- Collects and documents patient clinical history and assessment data
- Performs onsite testing (POC lab, EKG, phlebotomy, screening, fetal monitoring, spirometry, skin testing, etc.)
- Performs medical and nursing treatments per plan of care and scope of practice
- Performs procedures (dressings, wound care, minor/major procedures, catheter care, etc.)
