LPN; Med Pass

It is a 175 bed skilled nursing and rehabilitation facility located in West Babylon. Our mission is to provide the highest level of care for each of our residents. The ideal candidate will be experienced in performing a variety of care practices under the direction of the Registered Nurse and/or Unit Supervisor.

Responsibilities:

- Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs.

- Collaborate with physicians and other healthcare professionals to develop and implement patient care plans.

- Document patient information, medical history, and treatment plans accurately and thoroughly.

- Assist with case management and coordination of care for patients with complex medical needs.

- Conduct physical examinations and assessments to determine patient health status.

- Provide education and support to patients and their families regarding healthcare management.

- Administer basic life support and emergency care as needed.

Experience:

- Must have a valid LPN license in the state of NY.

- Experience working in a hospital or healthcare setting preferred.

- Proficiency in using electronic medical record systems, such as Athenahealth.

- Knowledge of medical coding and documentation practices.

- Familiarity with occupational health procedures and protocols.

- Ability to suture wounds and perform basic wound care.

- Experience working with individuals with developmental disabilities is a plus.

Standard shift:

• Day shift (3:30pm to 7:30pm)

Weekly schedule:

• Rotating weekends
