Position: Per Diem - Nurse Practitioner or Physician Assistant Suffolk County

Senior Community Care Coach – Nurse Practitioner / Physician Assistant (Per Diem)

$5,000 Sign‑On Bonus available to external candidates.

Job Summary

Senior Community Care (SCC) members provide care to patients in nursing homes, assisted living, and senior housing. The role delivers high-quality, cost-effective care and coordinates care transitions for Medicare and Medicaid members on a periodic or intermittent basis.

Primary Responsibilities
  • Deliver cost‑effective, high‑quality care to assigned members.
  • Manage both medical and behavioral, chronic and acute conditions in collaboration with physicians or specialty providers.
  • Perform comprehensive assessments and document findings in compliance with documentation requirements and CMS regulations.
  • Ensure accurate ICD‑10 coding and documentation to support diagnoses.
  • Complete initial, annual, and post‑hospitalization medication reviews and reconciliations.
  • Facilitate member plan‑of‑care implementation by engaging facility staff, families, and primary/specialty physicians.
  • Evaluate and revise the plan of care for effectiveness, necessity, and efficiency.
  • Utilize CCM practice guidelines and protocols.
  • Attend and complete all mandatory educational and Learn Source training requirements.
  • Travel between care sites as required.
Care Coordination
  • Understand payer/plan benefits and communicate them to providers, members, and key decision‑makers.
  • Assess medical necessity and effectiveness of ancillary services, communicate to providers and team.
  • Coordinate care transitions through different levels and settings.
  • Monitor member and family needs and adjust plans as situations change.
  • Review orders and interventions for appropriateness and response to treatment.
  • Evaluate plan of care for cost‑effectiveness while meeting member, family, and provider needs.
Program Enhancement Expected Behaviors
  • Maintain regular and effective communication with internal and external parties.
  • Promote the CCM program within assigned facilities through stakeholder partnership.
  • Show original thinking and creativity in developing improved methods and approaches.
  • Work independently and responsibly with minimal supervision.
  • Enter available hours into the web‑based application at least one month in advance.
  • Demonstrate initiative to achieve individual, team, and organizational goals.
  • Participate in CCM quality initiatives.
  • Check Optum email intermittently for required trainings, communications, and monthly scheduling.
Required Qualifications
  • Certified Nurse Practitioner through a national board.
  • Graduated from an accredited MSN program and board had certification through AANP or ANCC (preferred ANP, FNP, or GNP).
  • Active and unrestricted state license in the state of residence.
  • Active DEA licensure/prescriptive authority or ability to obtain post‑hire, per state regulations.
  • Ability to gain a collaborative practice agreement if applicable.
  • Physical ability to move a 30‑pound bag, navigate stairs, and handle varied dwelling conditions.
  • Availability to work 24 hours per month, including up to 16 hours off‑hours (after 5 pm, on weekends, or holidays). Max 960 hours per calendar year.
  • Valid driver’s license and reliable transportation to travel to client sites within a designated area.
Preferred Qualifications
  • 1+ year of post‑graduate experience within long‑term care.
  • Understanding of geriatrics and chronic illness.
  • Understanding of advanced illness and end‑of‑life discussions.
  • Proficient computer skills, including electronic medical record documentation.
  • Ability to develop and maintain positive customer relationships.
  • Adaptability to change.
Compensation and Benefits

Hourly pay ranges from $44.09 to $78.70 based on full‑time employment. In addition to salary, benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution. All benefits are subject to eligibility requirements.
