Horizon's Inpatient Hospice facility, The Kathy Hospice, is located in the rolling hills of the Froedtert West Bend campus and practices Horizon's hospice philosophy of celebrating the human spirit and providing quality and compassion care to our patients and their families.

In this role, you will play an important part in supporting and educating patients and their families as they embark on the end-of-life journey. You will draw on all parts of your experience: science and other knowledge from school, skills you've developed throughout your career, and most importantly: compassion and empathy.

We are seeking experienced nurses to join our Hospice Inpatient team.

Responsibilities include:

• Performing referral evaluations and admission assessments of patients and families seeking Hospice services.
• Performing physical, psychosocial, and spiritual assessments utilizing non-verbal cues and probes or modifying questions to obtain more comprehensive assessments.
• Communicating patient's and family's expectations of care, goals, and comprehensive learning needs to all members of the healthcare team via appropriate documentation.
• Providing Case Management functions as part of the IDG to an assigned case load.
• Providing services at quality levels consistent with professional standards of practice, approaching patients and families with care, concern, and compassion.
