Coverage Area: Travel throughout the territory is required—primarily Washington, Ozaukee, and northeast Milwaukee Counties
Schedule: Full Time Monday‑Friday 8 AM‑5 PM

Responsibilities- Assess and monitor physical, emotional, psychological needs of patients
- Create hospice care plans that align with patient wishes and goals
- Direct nursing care: administer medications, treatments, and interventions
- Provide pain and symptom management
- Educate and support the patient's family and caregivers
Collaborate with interdisciplinary team - Maintain accurate and timely documentation
- Participate in on-call rotation as required by local branch
- Graduate of an accredited nursing school with current licensure to practice in the state of operation
- One year of nursing experience required; three or more years preferred
- Experience in hospice or a similar setting preferred
- Knowledge of the hospice philosophy of care
- Commitment to clinical and documentation excellence
- Must be a licensed driver with an insured automobile in good working order
