Allay Hospice is seeking a compassionate RN Case Manager to join our hospice team in Wisconsin. You will provide patient-centered care with a dedicated after-hours triage support and collaborate with Social Work, Chaplaincy, Aides, and other providers.
The role offers a 8 AM–5 PM, Monday–Friday schedule with travel across the territory. A valid RN license and hospice or similar experience are preferred, with competitive benefits and a $5,000 sign-on bonus.
