In this role, you will:

• Develop individualized care plans based on client needs assessments, identifying areas for financial, physical, and emotional support.
• Assist clientsin enrolling in services like Medicare, food assistance, disability programs, and more.
• Advocate for clientsby connecting them to appropriate resources during times of crisis and providing ongoing personal support.
• Monitor client progress and adjust care plans as needed to ensure positive, sustainable outcomes.
• Maintain accurate case records, logging significant events and tracking client progress.
• Collaborate with internal programs and external community partners to provide comprehensive care and ensure clients receive all necessary services.

• Bachelor’s degree in social work or a related field (strongly preferred)
• Two years of experience in case management, family services, or social services is ideal
• Travel required– Valid driver’s license, reliable transportation, and vehicle insurance (minimum coverage: $100,000/person and $300,000/accident)
• Bilingual in English and Spanishis strongly preferred
