This program serves as a vital link between individuals in crisis and the services, resources, and community connections they need to thrive. What Will I Do as an Outreach Case Manager?In this role, you will:
- Develop individualized care plans based on client needs assessments, identifying areas for financial, physical, and emotional support.
- Assist clientsin enrolling in services like Medicare, food assistance, disability programs, and more.
- Advocate for clientsby connecting them to appropriate resources during times of crisis and providing ongoing personal support.
- Monitor client progress and adjust care plans as needed to ensure positive, sustainable outcomes.
- Maintain accurate case records, logging significant events and tracking client progress.
- Collaborate with internal programs and external community partners to provide comprehensive care and ensure clients receive all necessary services.
- Bachelor’s degree in social work or a related field (strongly preferred)
- Two years of experience in case management, family services, or social services is ideal
- Travel required– Valid driver’s license, reliable transportation, and vehicle insurance (minimum coverage: $100,000/person and $300,000/accident)
- Bilingual in English and Spanishis strongly preferred
