Overview
Delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation in coordination with other health team members. Effectively interacts with the patient, significant others, and other health team members while maintaining high standards of professional nursing. Provides high quality care to Geriatric, Adult, and Pediatrics populations.
Responsibilities- Perform bedside shift report at the beginning and end of each shift.
- Utilize hospital policies and procedures to keep patients safe from falls and other hazards.
- Assist in wound prevention and prevent hospital acquired pressure injuries.
- Obtain a comprehensive admission history and perform a physical assessment of each patient.
- Differiate between normal and abnormal assessment findings, analyze and prioritize abnormal findings, and initiate appropriate response.
- Anticipate actual or potential problems or health concerns in the hospital and/or home setting.
- Conduct ongoing assessment of patients’ psychosocial, physiological, and educational needs according to unit policy.
- Document subtle changes in patients’ physical and mental status, integrating diagnostic information into the assessment and care plan.
- Formulate individualized nursing diagnoses and plans of care, including discharge planning, and modify the plan based on ongoing assessment and changes in patient status.
- Administer prescribed treatments and medications, recognizing and questioning unsafe, incorrect, or inadequate orders and intervening to clarify orders.
- Teach patients the information necessary to achieve optimal self‑care during the delivery of patient care.
- Evaluate patients’ responses to care, assess comprehension of education, and document all aspects of the nursing process in a legible, concise, and timely manner.
- Communicate with patients, significant others, and the health care team, acting as a patient advocate and respecting patient rights and confidentiality.
- Advise on ethical concerns and communicate delays or wait times to patient or significant others.
- Use sound judgment and critical thinking skills to manage emergency and complex patient care situations.
- Demonstrate the ability to utilize and troubleshoot nursing equipment in accordance with unit and organizational policies.
- Monitor patient condition, document and communicate results to physicians as appropriate.
- Adhere to the six rights of medication administration, including IV fluids and enteral feeds, and monitor effectiveness.
- Assist patients with bathing, dressing, personal hygiene, mobilization, and ambulation.
- Measure and record vital signs, including height, weight, temperature, blood pressure, pulse, respiration, oxygen saturation, and pain.
- Provide patient and family education and coordinate nursing care for assignments.
- Participate in quality improvement activities, including quality assurance and monitoring.
- Delegate tasks to certified nursing assistants, patient service associates, and licensed practical nurses while ensuring completion of tasks.
- Prioritize patient care activities based on acuity and need.
- Coordinate and oversee nursing care provided to patients.
- Access resources for planning and coordinating patient care.
- Associate’s Degree (AA) in Nursing (ADN) required.
- Bachelor’s Degree in Arts/Sciences (BA/BS) or Bachelor’s Degree in Nursing preferred.
- Experience in hospital-based nursing or home health care preferred, but not required.
- Knowledge of nursing documentation standards and medical terminology.
- Knowledge of anatomy, physiology, pharmacology, and disease processes.
- Understanding of customer service principles and patient-centered care.
- Knowledge of health behavior, motivation, and psychological research methods.
- Ability to calculate medication doses and apply basic math skills.
- Excellent communication, listening, and interpersonal skills.
- Leadership and delegation abilities.
- Registered Nurse with Iowa Board of Nursing required.
- MOAB certification within 1 year of hire for all inpatient units.
- BLS within 60 days of hire.
- ACLS within 1 year of hire for Heart & Vascular units.
- Basic dysrhythmia and basic 12‑lead ECG within 6 months of hire for Heart & Vascular units.
