Description:
This position involves a dual role.
The Health Care Professional will be scheduled as House Supervisor and Transfer Center Coordinator.
Will be required to work shifts as House Supervisor during specific times.
House Supervisor:
Responsible for coordination and direction of activities on nursing units and other departments in the absence of Nurse Managers and Administration.
Exhibits professional behavior as a representative of nursing leadership.
Supervises staff providing general nursing care.
Works independently and functions as a team member.
Provides high-quality nursing services to patients, employees, administration, management, physicians, and visitors.
Maintains current knowledge of regulatory standards and evidence-based practices and ensures compliance.
Transfer Center Coordinator:
Facilitates the efficient transfer of patients between facilities.
Responsible for coordinating the safe, timely, and efficient transfer of patients.
Acts as a central point of communication between referring facilities, physicians, nursing leadership, bed management, and ancillary departments.
Ensures appropriate placement of patients based on clinical needs, bed availability, and organizational capabilities.
Ensures compliance with regulations and maintains detailed records for quality assurance.
The role requires coordination of communication between transferring and receiving medical staff.
Arranges required transportation and accommodations for patient transfers.
Maintains detailed records of all transfer activities for quality assurance.
Weekend and holiday rotation required.
