Spring Vale Health Centers is seeking a part-time or full-time Licensed Outpatient Counselor/Social Worker to provide assessments and counseling services to children, adolescents, and adults on an individual or family basis at our Carrollton Office.

Our multidisciplinary team includes counselors, psychiatrists, case managers, family practice providers, nurse practitioners, dental staff, pharmacists, and support staff. Strong relationships with community partners ensure individuals who use our services have access to the highest levels of care. We are nationally accredited by CARF International.
Minimum Requirements for Licensed Counselor/Social Worker:
- Current Ohio licensure as an LPC, LPCC, LSW or LISW.
- Pre-employment drug screen, background check, and reference checks are required.
Responsibilities:
- Provides assessment and planning services utilizing appropriate clinical skills and training. With input from client, family members or guardian establishes appropriate ISP treatment goals and objectives based on assessment information and establishes interventions, allowable services, and appropriate time frames.
- Executes counseling services utilizing appropriate clinical skills, training and treatment modalities, as based on identified goals, objectives and services.
- Completes intake forms and assessment updates within time frames established by policies, ODMH, and regulatory agencies. Assures clinical consistency between diagnostic assessment and identified needs, the ISP, and services received.
- Demonstrates clinical skills, knowledge and competence in utilizing program-related, research-based treatment approaches, general knowledge of psychotropic medications and potential side-effects.
- Advocates for the patient and demonstrates comprehension of all services offered to provide best service.
- Collaborates with community professionals, agencies and significant others as appropriate and documents in progress note.
- Demonstrates knowledge of community resources or refers clients.
