Job Overview
The Ambulatory RN - Care Coordinator is a vital member of the healthcare team responsible for ensuring seamless and high-quality care for patients in an ambulatory setting. This role involves both direct patient care and administrative responsibilities, with approximately 60% of time dedicated to patient-facing duties. The RN will support ambulatory physician clinics by seeing patients with complex and chronic disease that require additional support during outpatient visits.
They will be involved in working with patients to develop and execute care plans, provide patient education, and work collaboratively with interdisciplinary healthcare team members to optimize patient care and experience.

Qualifications- Minimum
Required:
Associate's Degree – Nursing. - Preferred:
Bachelor's Degree – Nursing. - Current Registered Nurse (RN) license in Ohio; BLS certification preferred.
- Minimum of 2 years of nursing experience in an ambulatory or outpatient setting.
- Engages in population‑appropriate communication.
- Has knowledge of growth and development milestones and tasks.
- Gives clear instructions to patients/family regarding treatment.
- Involves family/guardian in assessment, initial treatment, and continuing care.
- Identifies any physical limitations of the patient and deploys intervention when necessary.
- Recognizes and responds appropriately to patients/families with behavioral health problems.
- Interprets population‑related data and plans care appropriately.
- Identifies and responds appropriately to different needs based on unique psychological or cultural norms; performs treatments, administers medication, or operates equipment safely.
- Recognizes and responds to signs/symptoms of abuse or neglect.
- Acts as an expert clinician and role model in providing quality patient care within specialty area.
- Utilizes the nursing process for direct patient care and assists staff with assessment, planning, implementation, and evaluation.
- Conducts thorough assessments of patients’ health status during clinic visits and reassesses as needed.
- Documents nursing history and assessment for assigned patients, initiating referrals based on patient needs.
- Collects basic and advanced patient data; monitors trends that could impact health.
- Maintains awareness of comfort and safety needs from arrival to departure.
- Responds to life‑saving techniques per policies.
- Provides accurate hand‑off reports when patients move between ambulatory areas.
- Recognizes and reports changes in patient conditions and abnormal labs.
- Performs point‑of‑care testing as needed.
- Initiates and implements individualized care plans with other providers.
- Collaboratively develops patient care pathways to ensure consistent quality.
- Demonstrates competency in administering medications, wound care, and other interventions.
- Completes nurse visits and documents accurately and promptly.
- Assists with minor procedures and diagnostic tests.
- Monitors patients for condition changes, reports findings, and adjusts plans.
- Recruits patients for studies.
- Assists and collaborates with staff nurses in quality assurance.
- Introduces self to new patients and explains role.
- Prioritizes care based on individual assessment.
- Applies critical thinking in evaluating and implementing orders.
- Ensures timely nursing documentation with high accuracy.
- Maintains a professional and cooperative demeanor throughout interactions.
- Meets with patients and families to identify educational needs.
- Educates about health conditions, treatment plans, medications, and self‑care.
- Reassesses understanding and re‑educates as needed.
- Applies current literature in teaching techniques.
- Serves as expert patient educator for disease‑based and clinic‑based teams.
- Documents teaching and responses appropriately.
- Facilitates seamless transitions by coordinating referrals, scheduling follow‑ups, and ensuring timely access.
- Communicates effectively about test results, treatment plans, and follow‑up care.
