Post-Acute Care Physician (Part Time) opportunity in West Chester, PAGreat part-time (2 days per week) primary care role managing complex patient cases in a post-acute setting (SNF) in West Chester, Pennsylvania, focusing on improving care for some of the most vulnerable patients.

Qualifications:
Minimum of 1 year of experience, with a commitment to delivering high-quality, patient-centered care
Post-acute care experience is beneficial, though not required, as we provide comprehensive support and development opportunities
Active and unrestricted Pennsylvania license (or in process)
Active and unrestricted DEA/prescriptive authority (or in process)
A deep commitment to improving patient outcomes, leading clinical excellence, and driving change in the healthcare landscape
Internal medicine (IM), family medicine (FM), emergency medicine (EM), hospitalist or candidates with any related experience are welcome to apply
