• Conducting client assessments and reassessments as assigned
• Performing supervisory visits with clients
• Ensuring compliance with agency policies, state regulations, and care plans

• Active Licensed Practical Nurse (LPN) license in good standing
• Experience in home care or community-based services preferred
• Strong communication and organizational skills
• Reliable transportation and ability to travel to client homes
• Ability to work independently and professionally
