Your Health is seeking a Continuous Care Nurse to service our patients throughout the Columbia area. The role provides in-home, intensive, short-term bedside clinical care to manage uncontrolled symptoms and to stabilize and comfort patients in their homes, clinics, and care facilities, in adherence with laws and policies.
The Nurse Case Manager handles a full patient load, travels daily, and collaborates with an interdisciplinary hospice team.
