Nurse Practitioner, Care Management

Job Overview

The Nurse Practitioner, Care Management provides care management and primary care medical services to high-risk patients. The NP manages the assigned patient panel.

• Nurse practitioner with an active, unrestricted California license to practice medicine.
• Minimum 2 years of experience and training as a nurse practitioner.
• At least 1 year of experience in a high-risk specialty such as Geriatric, Hospice, or Palliative Medicine.
• Valid BLS certification or higher, following the American Heart Association (AHA) or American Red Cross guidelines.
• Experience with computer-based systems;
  Electronic Medical Record experience preferred.
