Alta Med is seeking a Licensed Vocational Nurse to develop and implement homecare services for program participants across contracted home services, durable medical equipment, and nursing home care. You will attend scheduled IDT and Coordination meetings to ensure integrated patient care.
The ideal candidate has an LVN license in California and at least one year of LVN experience, with strong understanding of elder patient needs and reliable transportation. A valid BLS certification is required.
