. UnityPoint at Home is seeking a skilled Registered Nurse (RN) to join our Care at Home Service Line in the Des Moines, IA area. This part‑time role (24 hours/week, Friday–Sunday) offers the opportunity to provide personalized, holistic care while practicing at the top of your RN license.

In this role, you’ll deliver innovative, home‑based population health services
, including:

  • Post‑Discharge Home Visits (PDHV) supporting safe transitions of care
  • Hospital‑to‑Home (HTH) visits focused on preventing avoidable readmissions
  • Advanced Care Planning (ACP) at Home to align care with patient goals and values
  • Primary Care at Home visits for proactive, relationship‑based care
  • Same‑Day Discharge (SDD) post‑operative visits at home
  • Additional value‑based population health services designed to keep patients healthier at home

As a UnityPoint at Home (UPaH) Care at Home RN
, you’ll work closely within an interdisciplinary team (IDT) to deliver non‑traditional, patient‑centered care as an extension of the UnityPoint Clinic (UPC) Ambulatory Division
. This collaborative model ensures seamless coordination across the continuum of care.

As an RN Visit Nurse, you’ll:

  • Assess patient and family needs, provide education, and coordinate individualized care plans
  • Deliver safe, outcome‑driven care using strong clinical judgment and critical‑thinking skills
  • Serve as a trusted advocate and clear communicator for patients and their support systems
  • Partner with interdisciplinary and ambulatory teams to ensure continuity, quality, and compassion in care delivery

Responsibilities

  • Patient and Family Needs:
    Assess the needs of patients and families, provide appropriate teaching, and make necessary referrals to ensure continuity of care.
  • Care Coordination:
    Integrate and coordinate care for at‑risk populations across the healthcare continuum.
  • Individual Care Plans:
    Develop personalized care plans using a collaborative approach with patients, families, and the healthcare team.
  • Quality and Safety:
    Ensure the quality and safe delivery of services according to the care plan, using equipment and supplies safely and efficiently.
  • Documentation:
    Promptly complete documentation to maintain an accurate legal record of patient care and time/travel entries.
  • Communication:
    Clearly and accurately communicate clinical status and plans to the interdisciplinary care team, administrative support, leaders, and physicians.
  • High‑Quality Care:
    Deliver high‑quality care using the nursing process, demonstrating knowledge and clinical skills specific to the assigned unit.
