This position is ideal for a nurse who is passionate about wound management, patient education, and helping individuals achieve the best possible outcomes in the comfort of their own homes.
As a Home Health Wound Care Nurse, you will provide specialized wound assessments, treatments, and ongoing clinical oversight while collaborating with physicians, patients, families, and the interdisciplinary care team. This position offers a flexible schedule and the opportunity to build meaningful relationships with patients while making a direct impact on their healing and quality of life.

- Perform comprehensive nursing assessments and develop individualized plans of care.
- Assess, monitor, and treat a variety of wounds, including surgical wounds, pressure injuries, diabetic ulcers, vascular wounds, and traumatic injuries.
- Perform wound measurements, staging, documentation, and ongoing evaluation of healing progress.
- Administer wound care treatments in accordance with physician orders and evidence-based practices.
- Educate patients, families, and caregivers regarding wound care, prevention strategies, nutrition, and disease management.
- Monitor for signs and symptoms of infection or complications and communicate changes in condition to the physician and care team.
- Coordinate care with physicians, therapists, aides, and other healthcare professionals.
- Complete Start of Care, Recertification, Resumption of Care, and Discharge assessments as assigned.
- Ensure accurate, timely, and compliant documentation within the electronic medical record.
- Participate in case conferences and interdisciplinary team meetings.
- Maintain compliance with all federal, state, and agency regulations.

- Current Registered Nurse (RN) license in the State of Iowa.
- Minimum one year of nursing experience required.
- Home health and/or wound care experience preferred.
- Strong assessment, critical thinking, and clinical documentation skills.
- Ability to work independently and manage a flexible schedule.
- Valid driver's license, reliable transportation, and current auto insurance.
- Current CPR certification.
