Job Overview

LTSS Service Coordinator – field‑based role located in Western Iowa (Polk, Ida, Cherokee, Pottawattamie, Story, Adair, Woodbury, Webster, Harrison, Shelby, Carroll, Crawford, O'Brien, Plymouth, Marion, Adams, Mills, Washington, Dickinson, Clay). Must reside in one of these counties or within reasonable commuting distance.

Key Responsibilities
  • Conduct face‑to‑face program assessments using standardized tools and motivational interviewing techniques.
  • Develop and document Person Centered Support Plans (PCSP) and backup plans.
  • Implement and manage individual waivers (LTSS, IDD, behavioral health, physical health) and coordinate with interdisciplinary care teams.
  • Serve as single point of contact for member’s health, behavioral, and long‑term service needs.
  • Manage non‑clinical needs for members with chronic illnesses or disabilities to ensure cost‑effective utilization of services.
  • Collaborate with member’s chosen care team (caregivers, family, service providers, physicians) to set short and long‑term goals.
  • Identify members who may benefit from alternative service levels or other waiver programs.
  • Submit utilization/authorization requests with supporting documentation aligned with care plans.
  • Report critical incidents to state and county agencies (Adult Protective Services, Law Enforcement).
  • Participate in appeals, fair hearings, grievances, and state audits.
  • Travel to client sites, work sites, and other locations as required.
Required Qualifications
  • Bachelor’s degree (BA/BS) in a health‑care related field.
  • Minimum two years of experience working with a social work agency or equivalent background.
  • Knowledge of state law, contract requirements, and federal regulations related to LTSS.
  • Strong interpersonal skills and ability to build collaborative relationships.
Preferred Qualifications
  • Experience with long‑term services and supports (LTSS) or behavioral health programs.
  • Knowledge of interdisciplinary care rounds and care coordination processes.
  • Previous field‑based or travel‑heavy role experience.
