Position Overview

The Special Education Transition Teacher (18–22 Program) provides individualized instruction and community-based programming for young adults preparing for life after high school. This role focuses on building independent living, vocational, and self-determination skills that support each student's transition into adult life, employment, and the community. Teachers design meaningful, real-world learning experiences while fostering independence, confidence, and self-advocacy. This position offers the opportunity to make a lasting impact while working within a highly collaborative educational team.

Responsibilities

• Plan and deliver individualized, functional instruction aligned with students' IEPs and post-secondary transition goals.
• Design and lead community-based instruction (CBI) in real-world settings such as job sites, grocery stores, banks, and public transportation.
• Adapt instruction to build independent living skills, including money management, cooking, hygiene, home management, and community navigation.
• Create a positive, respectful classroom and community environment where young adults are treated with age-appropriate dignity and independence.

• Develop trusting relationships with students and families as students prepare for adulthood.
• Support students' social-emotional development and self‑advocacy through positive behavior supports and crisis intervention when needed.
• Foster a culture of respect, encouragement, and high expectations appropriate for young adults.

• Coordinate and supervise job coaching, work‑based learning, and internship experiences.
• Teach job‑readiness skills such as resume writing, interviewing, workplace communication, and following workplace expectations.
• Partner with local businesses and community organizations to develop employment and volunteer opportunities.
• Collaborate with agencies such as the Bureau of Rehabilitation Services (BRS) and Department of Developmental Services (DDS) to support post‑graduation planning.

• Collect and analyze data on academic, vocational, behavioral, and independent living skill goals to guide instruction.
• Maintain accurate digital grade books and student records.
• Prepare quarterly progress reports and communicate student growth with families and outside agencies.

• Serve as an IEP Case Manager for students in the 18–22 program.
• Develop, implement, and monitor IEP goals centered on independent living, vocational, and post‑secondary outcomes.
• Lead or contribute to transition assessments, person‑centered planning, and Summary of Performance (SOP) documentation.
• Collaborate in developing Functional Behavioral Assessments (FBAs) and Behavior Intervention Plans (BIPs).
