Physical Therapist - Outpatient Rehab (Per Diem)
Location:
65 Memorial Rd, West Hartford, CT 05909.
Shift: Varying shifts covering clinic hours: 7am-7pm Monday–Thursday, 7am-5pm Friday. Hours determined by availability and clinical needs.
This per diem role covers our three West Hartford clinics and Bloomfield and Farmington locations. The Physical Therapist (PT) administers medically prescribed PT treatments to patients, evaluates functional level, determines treatment strategies, and delivers or delegates PT services to identify, prevent, remediate, and rehabilitate acute or prolonged dysfunction or pain.
Responsibilities- Upon physician referral, perform thorough examination and evaluation of patient’s level of function, including strength, movement, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function, vitals, endurance, gait, and functional abilities in activities of daily living.
- Establish measurable goals with patient/family based on appropriate patient care protocols and critical paths to meet patient needs.
- Implement, deliver, and delegate patient care using interventions such as modalities (heat, cold, light, water, sound, electricity), mobilization, and therapeutic exercise.
- Provide patient and caregiver education regarding treatment, precautions, and equipment use.
- Participate in patient transition planning and/or discharge planning, including recommendations to community resources and coordination with other care providers such as rehab therapists, nurses, social workers, and community resources.
- Delegate patient care tasks and provide clinical supervision for PT students, rehabilitation aides, and PT assistants.
- Bachelor’s, Master’s, or Doctoral Degree in Physical Therapy from an accredited program by the Commission on Accreditation in Physical Therapy Education (CAPTE).
- Open to new graduates.
- Previous experience as a Physical Therapist preferred.
- Current CT Physical Therapy license required or active temporary license at time of hire.
