, is seeking a dedicated and experienced Physical Therapist to join our team. We are committed to delivering high-quality care that promotes independence, mobility, and overall well-being for our patients as they recover in the comfort of their homes.

• Provide comprehensive physical therapy evaluations and treatments to patients in a home setting.
• Develop and implement individualized treatment plans tailored to patient needs and recovery goals.
• Collaborate closely with an interdisciplinary team, including nurses, occupational therapists, and physicians, to ensure a seamless transition from hospital to home.
• Educate patients and caregivers on therapeutic exercises, mobility techniques, and injury prevention strategies.
• Monitor patient progress, adjust treatment plans as needed, and document all care in compliance with state and agency regulations.
• Maintain a high standard of care, ensuring safety and effective recovery for each patient.

• Licensed Physical Therapist in the state of Connecticut.
• Home health or acute care experience preferred.
• Strong assessment, clinical, and communication skills.
• Ability to work independently while coordinating care within a multidisciplinary team.
• Reliable transportation to travel to patient homes within Hartford County.
