Practice Medicine the Way You Always Wanted To
At American Family Care (AFC), we're rewriting what it means to be an Advanced Practice Provider. Our hybrid Urgent Care + Primary Care model gives you the clinical variety that keeps medicine interesting — the fast pace and procedural mix of urgent care, balanced with the continuity and relationships of primary care. No insurance-authorization nightmares. No productivity treadmill that squeezes out the reason you got into medicine.
Exceptional patient care, a team that has your back, and a schedule that lets you have a life. If you're a Nurse Practitioner or Physician Assistant who wants to practice at the top of your license, grow your clinical skills, and actually enjoy going to work again — we'd love to meet you.

What You'll Do As a hybrid APP, you'll care for patients across the full spectrum of outpatient medicine — treating acute illness and injury one hour, managing a diabetic's A1C the next, and building long-term patient relationships along the way. You'll work collaboratively with physicians, fellow APPs, nurses, medical assistants, and radiology/lab staff to deliver efficient, high-quality care that keeps patients coming back (and referring their friends and family).
Day-to-day,you will:

- Order and interpret labs, imaging (onsite X-ray), EKGs, and point-of-care diagnostics; develop differential diagnoses and treatment plans
- Manage a broad range of acute presentations — lacerations, sprains and fractures, respiratory and GI illness, minor procedures, workers' comp injuries, and occupational health visits
- Provide continuity primary care: annual wellness exams, chronic disease management (hypertension, diabetes, asthma, hyperlipidemia), preventive screenings, and vaccinations
- Recognize and stabilize emergencies including cardiac events, shock, hemorrhage, seizures, anaphylaxis, and acute respiratory distress; coordinate transfers as needed
- Counsel patients and families on diagnoses, treatment plans, medication adherence, and lifestyle modifications in a way that actually makes sense to them
- Document clearly and accurately in the EMR (we're committed to charting systems that work with you, not against you)
- Collaborate with your supervising physician and care team to continuously improve patient outcomes and clinic flow
