Position: LPN/RN MDS Coordinator (Full-Time)

LPN/RN MDS Coordinator (Full Time 32 Hours)

Starting Pay:
Competitive rates based on experience.

Shift: Full Time (32 hours).

Responsibilities
  • The LPN/RN MDS Coordinator plans, executes, and oversees nursing services for residents.
  • Reports to the RN MDS Director.
  • Gathers information, assesses needs, establishes realistic goals, provides interventions, and incorporates interventions into an organized, concise, functional care plan.
  • Coordinates completion of comprehensive assessment by interdisciplinary team and includes recommendations in the written care plan for each resident.
  • Ensures all relevant issues for care of the resident are identified and goals to be accomplished for each problem or need are set.
  • Works with care planning team to implement final plans, encouraging residents and their responsible parties to participate in development and review of care plans, focusing on assisting residents to reach their highest practicable level of well‑being.
Key Duties
  • Ensures that nursing personnel are aware of each resident's care plan and that plans are used in daily nursing services.
  • Reviews nurses' notes and monitors residents to ensure plans are followed and needs met; assesses, reviews, and revises plans as required.
  • Plans, schedules, and conducts weekly care plan meetings for all residents according to OBRA and state requirements.
  • Completes the MDS with utmost accuracy and ensures highest level of reimbursement for the facility.
Requirements
  • Must hold a current state LPN/RN license and be a nurse in good standing.
  • Must meet all applicable federal and state licensure requirements.
  • Experience completing Minimum Data Sets (MDS) and resident care plans in long‑term care.
  • Knowledge of general, rehabilitative, and restorative nursing and medical practices, procedures, laws, regulations, and guidelines.
  • In-depth knowledge of nursing practices, medical procedures, and long‑term care regulations.
  • Strong attention to detail, excellent organizational skills, and ability to manage multiple tasks effectively.
  • Ability to instruct others and collaborate as a team player with prior experience in long‑term care.
  • Skills to maximize reimbursement and ensure Medicare compliance.
Key Requirement
  • Candidates without prior experience in MDS will not be considered for this position.
