Reports To:

Director of Social Services

• Implement and document a plan of care related to the resident’s social/emotional needs in the electronic medical record.
• Collaborate with nursing, MDS, Admissions and rehabilitation departments to facilitate discharge plans for residents.
• Inform and educate staff, residents, families, and responsible parties about the resident’s personal and property rights, and rights related to health services.
• Maintain updated resources for residents and families regarding community resources, service agencies, and organizations.
• Maintain a record of all reported concerns, assist in resolving them, and ensure follow-up and documentation.
• Be accessible to residents, family and staff for support and guidance.
• Provide timely referrals to appropriate agencies for resident/family assistance (e.g., conservatorship applications, Medicaid applications).

• Bachelor’s Degree from an accredited program in Social Work or a related field.
• Knowledge of admissions and discharge procedures and familiarity with residents’ rights and documentation requirements in accordance with current regulations and/or guidelines.
• Attention to detail, good follow-through skills, and ability to prioritize multiple tasks.
• Innovative with the ability to find creative ways to serve diverse resident needs while maintaining customer focus.
• Must have a vehicle and a valid driver’s license.
• Demonstrates cooperation and ability to work as a team.
• Patience, tact, enthusiasm and a positive attitude toward residents and their support networks.
• Ability to remain calm in stressful situations and communicate professionally with residents, staff, and family members.
