Rapid Rehousing Case ManagerOne of our clients is seeking a Rapid Rehousing Case Manager to provide service in the West Haven, CT area. The position is a full‑time client opportunity scheduled Monday‑Friday, 8:30–4:30.
Responsibilities- Identify the needs of clients and establish social services support during the transition to new housing.
- Determine eligibility for housing and assist with application for housing vouchers.
- Help clients find and secure apartments.
- Work with the homeless population to screen documents, including sensitive information.
- Assist with identification and documentation for securing basic needs and entitlements such as Medicaid, housing subsidies, food subsidies, etc.
- Initiate referrals for mental health and addiction treatment services as needed.
- Encourage social supports such as peer activities and groups and develop community connections.
- Provide employment support, including resume writing, job searching, and interviewing skills as appropriate.
- Continue to provide case management activities to newly housed clients after they receive housing to support their continued success.
- Bachelor’s Degree strongly preferred.
- Case management experience in supportive/transitional housing required.
- Compensation processed weekly.
