Job Overview:
We are seeking an experienced and compassionate Director of Social Services to lead the Social Services department in our Skilled Nursing Facility. This role is responsible for overseeing psychosocial services, supporting residents and families, and ensuring compliance with state and federal regulations.

Director of Social Services Key Responsibilities:

• Direct and oversee all Social Services functions within the facility

• Ensure residents’ psychosocial needs are assessed, documented, and addressed

• Provide support and counseling to residents and families as needed

• Coordinate discharge planning and transitions of care

• Ensure compliance with CMS, DOH, and facility policies

• Participate in care plan meetings and interdisciplinary team conferences

• Manage grievances and resident concerns in a timely and professional manner

• Supervise, train, and evaluate Social Services staff

• Support survey preparedness and respond to regulatory findings

Director of Social Services Qualifications:

• Bachelor’s degree in Social Work, Human Services, or related field (Master’s preferred)

• Prior Social Services leadership experience in a skilled nursing or healthcare setting required

• Knowledge of long-term care regulations and resident rights

• Strong communication, documentation, and organizational skills

• Licensed Social Worker preferred (state-specific, if applicable)
