Job Overview
Full‑time Clinician (Nurse Practitioner or Physician Assistant) providing in‑home health evaluations at clients’ residences across a defined coverage area.
Job HighlightsThe typical visit lasts approximately 45 minutes and includes a medication and medical history review, a physical evaluation, and additional diagnostic tests (e.g., diabetic eye exam, spirometry) if ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow; prescribing, ordering labs, or altering treatment regimens is not required.
Responsibilities- Conduct in‑home visits with compassionate care, building trusted relationships with patients.
- Travel locally and within licensed states, including overnight stays, car rentals, and/flying as required by the coverage area.
- Maintain flexible scheduling to meet the needs of members and the business.
- Perform virtual visits as needed to support patient care.
- Deliver all Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, bone density screening, and other products.
- Active, unrestricted license(s) in the required coverage area(s).
- Board certification (required).
- Ability to obtain and maintain multi‑state licenses as required.
- Availability to cover all counties within state and flexibility to extend to other areas of business need.

PayTypical pay range: $78,331.00 – $ (base hourly rate or base annual full‑time salary depending on experience, education, geography, and other factors).
