• Assess residents’ strengths and social, emotional, and mental health needs through communication with residents, their support systems, and facility staff, and help identify or coordinate the services needed.

• Collaborate with the interdisciplinary team to support resident rights, well-being, and understanding of the social services role.

• Participate in developing individualized, interdisciplinary care plans addressing each resident’s social service needs.

• Conduct meetings with residents and their support systems to provide guidance, resources and ongoing support to enhance quality of life.

• Manage discharge planning, including education, documentation, and referrals to community agencies and service providers.

• Build and maintain positive relationships with community-based organizations to support effective referrals and resource coordination.

• Bachelor’s degree in Social Work or a related human services field (e.g., sociology, gerontology, special education, rehabilitation counseling, psychology).

• At least one year of supervised social work experience in a healthcare setting working directly with individuals.

• Required state licensure/certification to practice in a long-term care environment.
