Position: PRN Speech-Language Pathologist (SLP)

Southwestern Nursing and Rehabilitation Center is dedicated to providing exceptional, compassionate care to our residents. Our in‑house therapy team works collaboratively to help residents regain function, improve quality of life, and achieve their highest level of independence.

Position Summary

We are seeking a PRN Speech‑Language Pathologist (SLP) to join our dynamic therapy team. The SLP will assess, diagnose, and treat residents with speech, language, cognitive‑communication, and swallowing disorders. This position plays a key role in supporting residents recovering from illness, injury, or managing chronic conditions.

Key Responsibilities
  • Conduct comprehensive evaluations of speech, language, voice, cognition, and swallowing functions.
  • Develop and implement individualized treatment plans tailored to each resident’s needs and goals.
  • Provide direct therapy services using evidence‑based practices.
  • Document evaluations, progress notes, and discharge summaries in accordance with facility and regulatory standards.
  • Collaborate with interdisciplinary team members (OT, PT, nursing, social services, etc.) to ensure coordinated, holistic care.
  • Educate residents, families, and staff on communication strategies and safe swallowing techniques.
  • Participate in care plan meetings and provide professional recommendations as needed.
  • Adhere to all state, federal, and facility policies and guidelines.
Qualifications
  • Master’s degree in Speech‑Language Pathology from an accredited program.
  • Current SLP license in the state of practice.
  • Certificate of Clinical Competence (CCC) from ASHA preferred.
  • Prior experience in a skilled nursing or rehabilitation setting is highly desirable.
  • Strong clinical assessment, communication, and documentation skills.
  • Ability to work independently and effectively manage a flexible PRN schedule.
