Case Manager

Responsibilities

• Provides case management and direct support to consumers both on site and in the community.
• Implements treatment plans, manages cases, and provides transportation and linkage.
• Ensures the appropriate completion of all related documentation.
• Functions as an essential member of the team, assisting in shift coverage, participating in program activities, and communicating with team members to cover all applicable duties.

• BSW in social work, BA/BS in psychology, or a related field.
• Two years of experience in mental health and human services.
• Valid Driver’s License.

Full-Time

$40,000 - $43,000 annual salary

Sunday thru Thursday or Tuesday thru Saturday. 3 days shifts, 2 evening shifts.
