RWJBarnabas Health, Primary Care Service Live is seeking a Full‑Time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to be part of the primary care service line providing coverage to northern New Jersey.

  • Master's degree required or Graduate Level Certification
• APNs
  • Current NJ APN
  • Nurse Practitioner Certification
  • RN License

• Performs comprehensive initial and episodic health assessments to identify existing or potential health problems
• Initiates and implements an individualized nursing and medical plan of care in collaboration with interdisciplinary team members
• Initiates appropriate diagnostic, therapeutic, medical and nursing actions in conjunction with a physician
• Provides follow‑up care including acute and routine examinations
• Assesses progress, identifies problems, initiates preventive measures, recognizes emergent problems, initiates proper care in collaboration with a physician
• Monitors and adjusts patients’ medication according to protocol or medical plan of care
• Implements nutritional therapies, as appropriate
• Triages patient problem calls which require medical attention
• Orders appropriate laboratory and diagnostic procedures
• Recognizes normal and abnormal laboratory findings and the involved pathology
• Documents information in patients’ charts following documentation standards
• Provides health education to the patient and family (e.g., management of side effects, anticipatory teaching)
• Manages medical emergencies as indicated, and according to standing orders
• Provides monitoring and continuity of care between health care settings
• Provides patient and family counseling and reassurance regarding psychological or psychosocial issues
• Initiates proper referrals to private or community agencies
• Acts as a resource and teacher for other members of the health care team
• Shares knowledge and expertise with staff
• Collaborates with nursing and medical staff at partner and affiliate sites to coordinate continuity of care
• Evaluates the quality of care as well as the quality of life for patients through assessment of patient response and recommends interventions, as indicated

• Provide both onsite and telemedicine support and coverage for practices as needed
• Cover all practice sites within the assigned regional territory
• Receive mileage reimbursement for travel to assigned locations from the primary department
• Frequency of travel will be contingent on the practice need for the assigned location
• Duration of coverage will be contingent on the practice need for the assigned location
