On Call Hospice RN-Nights

We are seeking a compassionate and dedicated On-Call Hospice Registered Nurse (RN) to join our team. This role is crucial in providing after-hours care and support to hospice patients and their families. The ideal candidate will respond to patient needs during night shifts, ensuring high-quality, end-of-life care in accordance with the hospice philosophy.
Responsibilities:
- Provide nursing care to hospice patients during after-hours, addressing physical, emotional, and spiritual needs.
- Respond promptly to patient calls, perform on-site visits as needed, and manage urgent patient needs.
- Assess and manage pain and other symptoms, providing comfort and relief to patients.
- Offer guidance and emotional support to families during challenging times.
- Communicate effectively with the interdisciplinary hospice team, including physicians, social workers, and chaplains.
- Maintain accurate and timely records of patient care and communication.
- Follow VNA Health Group's hospice best practices, ensuring compassionate and patient-centered care, effective communication, and continuous quality improvement.
Qualifications:
- Current NJ RN license
- Minimum of 1-2 years of hospice or palliative care experience preferred.
- Strong assessment, critical thinking, and communication skills.
- Ability to work night shifts, including weekends and holidays as scheduled.
- Reliable vehicle, valid driver's license, and proof of auto insurance.
Working Conditions/Physical Demand:
Working indoors and outdoors with clinical equipment and use of phone and computer;

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $43.00 to USD $51.00.
