We are seeking a Spanish-speaking NP or PA to support our growing Value-Based Care and ACO initiatives. This position combines telemedicine and traditional primary care, partnering closely with the quality team to proactively address care gaps, improve Star Ratings, and support patient outcomes across multiple payer programs.
Responsibilities- Conduct telemedicine visits for established patients identified through ACO and value-based care gap reports.
- Work closely with the Care Quality team to address preventive care, chronic disease management, and quality measure gaps.
- Serve as an extension of patients' PCPs by engaging patients who are unable or unlikely to present for routine office visits.
- Spend 2 full days per week performing telemedicine visits with dedicated support staff at an office location
- Rotate among primary care practices during the remaining 3 days per week
, functioning as a float provider. - See approximately 18-22 patients per day
. - Leverage strong administrative support including scribes, triage staff, and care coordination resources.
- Maintain high-quality documentation and support organizational goals tied to value-based reimbursement and shared savings programs.
- Schedule:
Monday-Friday, 8:00 AM-5:00 PM

- Active NJ Nurse Practitioner or Physician Assistant license.
- Primary care experience preferred.
- Understanding of preventive care, chronic disease management, and quality measures.
- Comfortable with both telemedicine and office-based patient care.
- Spanish fluency required.
- Strong communication skills and commitment to population health initiatives.
