The Senior Teen Outreach Program Facilitator is accountable for providing leadership and operational support to ensure the successful implementation and sustainability of the Teen Outreach Program (TOP). Working in partnership with the TOP Coordinator, this role supports program quality, facilitator development, training coordination, and continuous improvement efforts across CHS TOP clubs. Overall, the Senior Teen Outreach Program Facilitator contributes to CHS’s strategic objective of reimagining what true community impact means by creating the foundation for every child, every dream, every future.

1. Provides leadership and operational support to ensure the successful implementation and sustainability of the Teen Outreach Program (TOP). Working in partnership with the TOP Coordinator, this role supports program quality, facilitator development, training coordination, and continuous improvement efforts across CHS TOP clubs.

  • Coordinate, schedule, and handle all logistics for successfully implemented Teen Outreach Program® Facilitator trainings.
  • Communicate to Wyman technical assistance needs of Provider Organizations and Facilitators as determined through site visits.
  • Maintain professional and open communication with the TOP replication partner and Wyman.
  • Deliver weekly TOP sessions at designated school sites per fidelity standards, know and deliver lessons with accuracy.
  • Coordinate community service learning (CSL) opportunities and provide overall support to enrolled teens.
  • Distribute Pre/Post surveys to consented teens and complete facilitator logs.
  • Enter consented and enrolled teens into Health Beginning Data System (HBDS), Wyman Connect, and/or CIS Connect, as applicable.
  • Discharge inactive teens from Wyman connect, CIS Connect and update the discharged teens log.
  • Recruit teens by actively recruiting candidates currently seeking jobs and utilizing passive recruiting methods to attract talent who are not searching but open to opportunities.
  • Keep track of attendance, ensure attendance meets fidelity standards, and notate/communicate change in attendance.
  • Transport consented teens off-site for events and projects.
  • Distribute appropriate permission slips to teens for community service learning CSL events and projects.
  • Complete quarterly reports and all required trainings by designated deadlines.
  • Co-facilitate with a teammate or assume facilitation responsibilities as a substitute when necessary.
  • Report any challenges to coordinator and collaborate regarding cancellation and/or rescheduling of clubs.
  • Shop monthly for snacks and clean designated storage area.
  • Shop for supplies for CSL projects, keep track of receipts, and ensure use of tax exempt at all times for company purchases.
  • Other duties as assigned may include communication with clients, join meetings, mandatory updates and reporting to partners and funder, ensure that the program remains in accordance with contract and deliverables, fully complete the monthly PPR document, monthly upload of the Wyman Fidelity Report.
  • Attend Coordinator’s Exchange and other professional development required of the coordinator.
