Job Title: RN / LPN MDS Coordinator

Facility: Beach Breeze Rehab & Care

Location: West Palm Beach, Florida

Schedule: Full-Time | Monday-Friday, 8:00 AM - 4:30 PM

Pay Rate: $36.00-$46.00 per hour

Beach Breeze Rehab & Care is seeking an experienced and detail-oriented RN or LPN MDS Coordinator to join our interdisciplinary leadership team. This key role is responsible for overseeing the accurate and timely completion of MDS assessments while ensuring compliance with all state and federal regulations. The ideal candidate is organized, collaborative, and committed to maintaining excellence in resident-centered care.

• Coordinate and complete all MDS assessments in accordance with CMS and state regulations
• Ensure accurate implementation of RAPS, triggers, and care planning processes
• Maintain and update MDS and care plan policies and procedures
• Monitor and manage assessment schedules to ensure timely submission
• Track Quality Indicators (QI) and Quality Measures (QM) and implement corrective actions as needed
• Collaborate with the interdisciplinary team, including nursing, therapy, social services, and dietary
• Assist with discharge planning by coordinating care plan components
• Participate in facility surveys, audits, and inspections
• Support quality assurance and performance improvement initiatives
• Facilitate interdisciplinary care plan meetings
• Communicate effectively with residents, families, and external care providers
• Evaluate residents’ functional abilities to support therapy and activity planning

• Active Florida RN or LPN license (required)
• Minimum 2 years of nursing home experience preferred
• Strong knowledge of MDS, PPS, QI/QM, and care planning processes
• Excellent organizational, communication, and teamwork skills
• High level of professionalism, integrity, and accountability
