On Call-Professional; Medical Social Worker

Position: On Call-Professional (Medical Social Worker)
This 12-month position facilitates medical social work services through the provision of social service triage, biopsychosocial assessment, crisis intervention, and care coordination for assigned patients of the Palm Beach County Fire Rescue (PBCFR) Mobile Integrated Healthcare Program (MIHP), both in patients' homes and via telehealth. Liaises with PBCFR, local social service agencies, and healthcare providers in relationship to the MIHP.

Reports directly to the Medical Social Work Coordinator. The employee is expected to be flexible, independent, and able to initiate/follow-up on patient care related to assigned patients, within the MIHP.

QUALIFICATIONS

This position requires qualified applicants to possess the following:

• Master's Degree in Social Work; minimum of two (2) years of medical social work experience.

PREFERRED QUALIFICATIONS

Preferred qualifications include:

• Licensed Clinical Social Worker (LCSW) with experience in medical social work, in homecare, acute care/hospital, hospice, or other healthcare settings
