Admissions Coordinator

Trustbridge Hospice, a part of Empath Health, is seeking a compassionate and customer-focused Admissions Coordinator to join our team in West Palm Beach. This position is onsite in a call center environment.

This full-time position works Monday through Thursday, 10:00 a.m.

- 7:30 p.m. supporting the admissions process and ensuring a seamless, empathetic experience for patients and families during one of life's most meaningful transitions.

Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.

What You'll Do

* Facilitate the conversion of referrals to admissions through completion of the intake process.

* Provide exceptional customer service to patients, families, and referral sources with empathy and professionalism.

* Communicate effectively with interdisciplinary team members to ensure timely and accurate admissions.

* Maintain accurate records and documentation in accordance with organizational policies.

What You'll Need

* Associate degree preferred.

* Three (3) years of experience in healthcare administration or customer service.

* Knowledge of hospice or home health programs preferred, with willingness to develop clinical understanding.

* Ability to handle a high call volume.

* Strong communication, organization, and computer skills.
