We are seeking a Clinical Team Leader to join our dynamic team in delivering outstanding healthcare services. Located in the heart of our community, this role is pivotal in enhancing patient outcomes and supporting our mission of excellence in senior care.

• Lead and manage clinical teams to ensure high-quality patient care.
• Oversee patient assessments and develop individualized care plans.
• Collaborate with interdisciplinary teams to enhance service delivery.
• Ensure compliance with healthcare regulations and standards.
• Provide mentorship and training for nursing staff.
• Monitor patient progress and adjust treatment plans as necessary.
• Facilitate communication between patients, families, and healthcare providers.
• Participate in quality improvement initiatives.

• Registered Nurse (RN) license required.
• Proven experience in critical care or long-term care settings.
• Strong leadership and team management skills.
• Excellent communication and interpersonal abilities.
• Familiarity with electronic medical records (EMR) systems, particularly Epic.
• Knowledge of healthcare regulations and compliance standards.
• Experience in utilization management is a plus.
• Certification in Basic Life Support (BLS) preferred.
