Summary:
The Senior Living Clinical Care Coordinator (RN) serves as the on‑site clinical representative of Health at Home within a partnered senior living community. This hybrid role blends direct clinical expertise with relationship‑building, care coordination, and community engagement. The Senior Living Clinical Care Coordinator acts as the primary clinical point of contact for residents, families, community staff, and providers - ensuring seamless coordination, continuity of care, and an exceptional experience for those we serve.
EssentialJob Functions / Responsibilities:

On‑Site Clinical Support & Resident Care: Serve as the on‑site clinical point of contact for residents, families, and community staff. Assess and coordinate residents’ needs for home health, therapy, and private duty services. Conduct nursing assessments, medication pours, and skilled nursing visits as needed. Participate in care planning with internal teams, providers, families, and community staff. - B.
Community Relationship Building & Representation: Build and maintain strong relationships with residents, families, and community leaders. Represent Health at Home at events, meetings, and gatherings. Host health education sessions, wellness checks, and engagement activities. - C.
Oversight of Private Duty Services: Provide training, oversight and support to private duty caregivers within the community. - D.
Documentation, Compliance & Standards: Maintain compliant documentation following Health at Home and regulatory standards. Uphold clinical, operational, and safety requirements. - E.
Service Line Growth & Community Impact: Support growth of home health and private duty service lines within the community. Promote trust and partnership through professional presence. - F.
Additional Duties and Organizational Support The employee will perform additional responsibilities as needed to support resident care, clinical operations, regulatory compliance, and the overall success of Health s may include assisting with emergent care needs, participating in quality or performance‑improvement efforts, supporting community‑based initiatives, and contributing to projects, tasks, or functions that fall within the scope of the organization’s mission and standards.
Qualifications:
- Active RN license in Florida.
- 1–3 years nursing experience in home health, senior living, or care management.
- Excellent interpersonal and communication skills.
- Commitment to serving older adults.
- Prior liaison or community‑facing nursing experience preferred.
- Valid driver’s license and auto insurance.
