Empath Trustbridge Hospice, is seeking a compassionate and motivated Registered Nurse (RN) to provide Continuous Care overnight visits for patients and families in Palm Beach County. This role is ideal for an RN who thrives in one-on‑one patient care and finds purpose in delivering comfort, dignity, and peace during life’s most vulnerable moments. Your shift would be: 7pm - 7:30am and every other weekend.

What you'll DoAs an RN with Empath Trustbridge Hospice, you will play a vital role in ensuring patients and families feel supported, informed, and cared for during urgent and often emotional situations.
- Support and uphold the organization’s mission, vision, and values in every interaction
- Coordinate patient and family care in collaboration with the attending and hospice physicians
- Perform comprehensive patient and family assessments to identify physical, emotional, and psychosocial needs
- Develop and manage the Interdisciplinary Group (IDG) Care Plan
- Ensure timely implementation and coordination of all interventions and services outlined in the care plan
- Respond promptly and effectively to emergent needs and concerns during field visits
- Deliver a consistently high level of personalized, compassionate, and engaging care
- Advocate for patient comfort, dignity, and quality of life at all times
- Foster trust and reassurance to ensure the highest level of patient and family satisfaction
- RN degree from a college or university, accredited by the Accreditation Commission for Education in Nursing and/or the Commission on Collegiate Nursing Education.
- Registered Nurse currently licensed in the State of Florida.
- BSN preferred
- Hospice certification preferred.
- Current Basic Life Support Certification.
- Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
- Basic computer skills and EMR software experience a plus.
- Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
