Conviva Senior Primary Care is a large senior-focused, value-based care provider. As a Nurse Practitioner (NP) or Physician Assistant (PA), you will be part of a collaborative multidisciplinary team delivering personalized, high-quality care at our Conviva West Palm Beach Center. You will report to the Associate Medical Director.

- Provide comprehensive care to seniors following clinical standards.
- Collaborate daily with care team to ensure coordinated, high-quality care.
- Manage referrals, hospital/SNF coordination, DME, and home health services.
- Document care accurately and efficiently with support from quality-based coders.
- Participate in on-call rotation and support growth plans at your center.
- Have a passion for providing value-based care.
- Master's Degree in Nursing or completion of a PA program with board certification.
- Active, unrestricted NP or PA license in the state of practice.
- Screening for TB required if selected for this role.
- Experience in value-based care, primary care or Medicare Advantage settings.
- Active DEA license and Medicare/Medicaid provider numbers or ability to obtain them.
- Familiarity with HEDIS quality indicators and Medicare guidelines.
- Spanish and English language fluency.

Language Proficiency RequirementAssociation who speaks with a member in a language other than English must take a language proficiency assessment. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.
Weekly Hours40 hours
Compensation$123,800 - $170,300 per year
This job is eligible for a bonus incentive plan based on company or individual performance.
