Nurse Practitioner

Overview

Join a team redefining senior primary care with Humana's Primary Care Organization. As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, multidisciplinary team dedicated to delivering personalized, high-quality care for seniors.

Scheduled weekly hours: 40

$123,800 – $170,300 per year; the compensation range may vary based on location and individual qualifications. This position is eligible for a bonus incentive plan based on company and/or individual performance.

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care accurately and efficiently with support from quality-based coders.
• Participate in on‑call rotation and support growth plans at the center.
• Demonstrate passion for providing value‑based care.

• Master's degree in Nursing or completion of a PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• Will be required to undergo TB screening upon selection.

• Experience in value‑based care, primary care, or Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers, or ability to obtain these licenses and numbers.
• Familiarity with HEDIS quality indicators and Medicare guidelines.
• Fluency in Spanish and English.

Any associate who speaks with a member in a language other than English must take a language proficiency assessment provided by an outside vendor. Applicants must also complete the Interagency Language Rating (ILR) test as provided by the Federal Government.
