Position Description
Bluebird Kids Health is seeking a dedicated and compassionate Nurse Care Manager to join our dynamic CARES Center team. The CARES Center serves as our enterprise access and remote care hub, delivering essential resources, education, and support to children and their families. The Nurse Care Manager will play a critical role in supporting medically complex pediatric populations through coordinated, patient-centered care.
This role aligns with Florida Medicaid (AHCA) and HEDIS quality standards, focusing on reducing avoidable emergency department (ED) utilization, improving chronic disease outcomes, and ensuring timely follow-up care. Reporting to the Senior Manager, Clinical Services, you will collaborate closely with our Provider team to provide seamless care coordination and clinical support, ensuring exceptional healthcare delivery across our pediatric clinics. Your contributions will directly support our mission to provide exceptional care so every child can thrive.
What You’ll Do
Care Management- Provide chronic disease and complex care management for pediatric patients, including but not limited to Asthma, ADHD, and Obesity.
- Perform timely post-discharge and post-ED outreach to re-engage patients in care and ensure continuity with our patient-centered medical home.
- Schedule and ensure completion of post-acute follow-up visits within established time frames (e.g., within 30 days of a chronic disease–related ED or inpatient visit).
- Develop, implement, and update individualized, family-centered care plans.
- Educate and coach patients and families on self-management, at-home strategies, and medication adherence.
- Proactively track patient outcomes such as acute exacerbations, ED/urgent care utilization and hospitalizations.
- Manage quality measures tied to Patient Centered Medical Home (PCMH) accreditation and Medicaid value-based contracts.
- Close care gaps such as ED follow-ups, well-child visits, and condition-specific measures.
- Participate in and help drive quality improvement initiatives.
- Maintain accurate, timely, and comprehensive documentation for all patient interactions in the EHR.
- Document and track ED visits, follow-ups, and chronic disease interventions.
- Ensure compliance with Florida Medicaid requirements.
- Ensure adherence to internal policies, HIPAA regulations, and applicable local, state, and federal healthcare standards to guarantee a safe and compliant healthcare environment.

What You’ll Need
Required:
- Bachelor's degree in nursing preferred or 5-10 years of work experience.
- BLS Certification.
- Unencumbered Florida Registered Nurse (RN) or Licensed Practical Nurse (LPN).
- Minimum 5 years of clinical experience.
- Care Management Experience.
- Value Based Care experience preferred.
- Strong communication and interpersonal skills with the ability to engage effectively with patients, families, and the healthcare team.
- Excellent organization and time-management skills to handle multiple tasks and priorities efficiently.
- Bilingual proficiency in English and Spanish is strongly preferred.
- Experience with pediatric populations is strongly preferred.
Preferred:
- Medicaid or pediatric experience.
- Knowledge of HEDIS measures and Patient Centered Medical Home Accreditation.
